The client agrees, through sending the request, that they agree and consent to voluntarily participate in the given offers/activities, assuming all risks and injuries involved and being released of any liability. Everything will be based on a solid understanding for the client in participation of the protocol proposed, with DS Fitness Health Nutrition given the right to stop the client at any given time, should any incident seem to be harmfully noted by observation of symptoms/diagnostics, and/or abnormal response while performing any exercise, or during treatment in any of the therapeutical sessions.

By sending out the request form for any services displayed, the client and DS Fitness Health Nutrition agree that no extra charge will be invoiced for the first initial contact request.
The purpose of the sent request is to obtain a better understanding of what the (potential) client/s overall physical aim is to be achieved.
The request serves to offer a potential collaboration only after DS Fitness Health Nutrition consented to render the services to the specific goal the client focuses on achieving.
